Inhaltliche Zusammenfassung

Arzneimittelrückstände in der Umwelt stellen ein zunehmendes Problem dar. Immer mehr Wirkstoffe werden in verschiedenen Umweltkompartimenten nachgewiesen. Grundsätzlich scheint eine Intervention zu Beginn des Lebenszyklus von Arzneimitteln eine langfristige und kosteneffiziente Methode zur Minderung des Umwelteintrages von Arzneimittelrückständen verglichen mit End-of-pipe Ansätzen wie des verbesserten Rückhalts in Klärwerken zu sein. Ein vielversprechender Ansatz hierfür ist ein Informationssystem, das Wirkstoffe anhand ihrer Umweltschädlichkeit klassifiziert, Informationen zum Umweltverhalten weitergibt und praktisch bei den Therapieempfehlungen für (Fach-)Ärztinnen*Ärzte und Apothekerinnen*Apotheker zur Verfügung steht. Schweden ist ein Land mit Vorreiterrolle, das schon seit etwa zwei Jahrzehnten mit einem solchen System arbeitet. Die Datenbanken fass.se der pharmazeutischen Industrie und janusinfo.se der Region Stockholm enthalten Informationen zum Umweltverhalten von Wirkstoffen, welche aktiv in Therapie- und Wirkstoffempfehlungen der "Wise List" einfließen. Dadurch existiert eine Kombination aus Umweltinformationssystem (Datenbank zum Umweltverhalten von Wirkstoffen), Klassifikationssystem (Einstufungen des Umweltverhaltens nach gewissen Parametern) und Vermittlungssysteme (Webseiten und "Wise List") durch Integration in die Therapieempfehlungen. Neben Schweden arbeiten auch Finnland und Schottland mit ähnlichen Systemen. Ziel dieser Machbarkeitsstudie ist es, die Übertragbarkeit und Anpassung an das deutsche Gesundheitssystem zu prüfen. Hierfür werden der Aufbau und die zentralen Stellschrauben der Systeme dieser drei Länder analysiert. Im Fokus stehen dabei technische, organisatorische, rechtliche, politische und finanzielle Aspekte sowie eine Wirkungsanalyse. Zusätzlich werden daraus Schlüsse gezogen, inwiefern eine Übertragbarkeit und Anpassung an das deutsche Gesundheitssystem in Bezug auf die technische, organisatorische, rechtliche, politische und finanzielle Machbarkeit sowie Wirkungsanalyse gegeben ist. Berücksichtigt werden Positionen von Stakeholdern des deutschen Gesundheitssystems zu einem solchen System. Anschließend wurden die Erkenntnisse genutzt, um verschiedene Ansätze der Integration in das deutsche Gesundheitssystem zu erarbeiten und ein eigenständiges Konzept für einen "Arzneimittelindex Umwelt" entwickelt. Dieses umfasst ein deutsches Informations-, Klassifikations- und Vermittlungssystem, welches im Rahmen der geltenden Rechtslage und angepasst an die Strukturen des deutschen Gesundheitssystems integrierbar ist.
Pharmaceutical residues in the environment are a growing problem. Increasing concentrations of various active ingredients are detected in different environmental compartments. In principle, intervention at the beginning of the life cycle appears to be a long-term and cost-efficient approach to reduce the input of active ingredients into the environment when compared with end-of-pipe approaches such as enhanced wastewater treatment. One promising approach, for example, is a pharmaceutical index for the environment: an information, classification and dissemination system that classifies active ingredients according to their harmfulness to organisms in the environment, passes on information on environmental behavior and practically incorporates it into therapy recommendations for (specialist) physicians and pharmacists. The aim is for decision-making to take place upstream and for medical and pharmaceutical staff to automatically integrate the sustainability aspect into guideline-compliant treatment. Sweden is a pioneering country that has been working with such a system for around two decades. The databases fass.se of the pharmaceutical industry and janusinfo.se of the Stockholm region contain information on the environmental behavior of active substances, which is actively incorporated into therapy and active substance recommendations in the 'Wise List', a guideline containing about 200 pharmaceuticals for the treatment of 80% of the most common diseases. This provides a combination of an environmental information system (database with information on environmental behavior of pharmaceuticals), classification system (Classification of environmental behavior according to certain parameters) and dissemination system (websites and Wise List) through integration into the therapy recommendations. In addition to Sweden, Finland and Scotland work with or on similar systems too. The aim of this feasibility analysis is to examine the transferability and adaptation to the German healthcare system. To this end, the structure and central levers of the systems in these three countries are analyzed. The focus is on technical, organizational, legal, political and financial aspects as well as an impact analysis. In addition, conclusions are drawn on the extent to which transferability and adaptation to the German healthcare system is possible in terms of feasibility of the beforementioned aspects and an impact analysis. The positions of stakeholders in the German healthcare system on such a system were taken into account. The findings from this were then used to provide perspectives on an information, classification and dissemination system and to develop various approaches for integration into the German healthcare system. It was found that it is not possible to fully transfer the systems analyzed in the case studies due to the differences between healthcare systems. However, an independent concept for an 'Arzneimittelindex Umwelt' was developed on this basis. This concept comprises a German information, classification and dissemination system that can be integrated within the framework of the current legal situation and adapted to the structures of the German healthcare system.
